Patient dismissal letter serves as formal document of patient’s discharge from the hospital, medical centre or from the treatment of a particular doctor. It is one of the most common documents written in the medical practice. It is usually issued after the doctor declares that the treatment has been completed and no other treatment is required for the patient. It serves as a formal notice for discharge of patient.It is also issued when a patient requires specialized treatment.
