Hi,

This is to inform you all that I am planning to participate in a 360 degree feedback program. This would involve feedback about how my co-workers and managers perceive me in my office. I would invite you to please be a part of my feedback team. To participate in the process, all you need to do is take an online survey which would take 10-15 minutes of your time. Make sure that you are honest in your responses and these responses will be them processed by the team of HR personnel. The team is going to prepare a final feedback report. The team is going to maintain the confidentiality and anonymity of all the responses except that of the manager which will be mentioned in the report.

Please inform me in case you are unable to be a part of this feedback process. You are otherwise going to receive a mail from the HR department which will comprise of all the instructions, process of taking the survey and contact details for further assistance.
