A leave letter is written and submitted at the workplace in advance if you are scheduled for a doctor’s appointment. It helps you to inform your boss of your pending work and meetings and gives time to make arrangements so that your work is covered and submitted on time.
In this post are some examples of leave application due to appointment with doctor for checkup, office leave for doctor appointment and sample leave application. You may use the leave application format for letter for hospital checkup to draft a customized letter.

Subject: Request of leave due to appointment with doctor for checkup
Sir,
I hope you are in good health and life is treating you well. I am Samantha Grey, the production manager at your good company.
Health is an important part of one’s lifestyle and cannot be ignored. Due to all the hustle and bustle of this fast pacing life, I have not really been able to pay attention to my health which has resulted in extreme physical and mental exertion.
I cannot really move forward with this state of health and need to see a doctor immediately. I have my appointments scheduled with my doctors for the same. Kindly grant me leave for a period of two days to have an exam and recuperate appropriately.
Thank you for your time and cooperation.
Yours truly,
Samantha Grey
