Dear Mr. Smith,

This is to inform you that you have failed to complete the project that was assigned to you by your manager on the decided date. Not only this, you have also found guilty of submitting wrong reports related with the project which have mislead the team and have interfered with timely completion of the task that you allotted to you.

Your casual behavior towards company projects is a serious offence as it is hampering the image of the organization and you are setting a wrong example for your juniors and co-workers. Such kind of negligence will not be tolerated at any cost.

Please consider this as a final warning as it is against the rules of the company to engage in personal chats during working hours and to be absent from the site for personal reasons. These are the reasons because of which you have not been able to deliver the performance as expected from you. From today onwards, you are expected to correct your actions or else the management will have to take strict actions against your casual attitude.

In case you have any reasons supporting your actions then please feel free to contact me at 12345-678.

Sincerely,

Jacob Brown
